Severe pre-eclampsia does not wait. In this case, minutes count and delay means direct risk to mother and baby.<\/p>\n<p>The standard for this work is clear. The patient is rapidly evaluated, necessary consultations are made, the decision is not delayed, and if necessary, the patient is stabilized and referred to a higher center. The Ministry of Health Emergency Obstetric Care Management Guide also states this. It does not mandate the presence of a perinatologist in every hospital, but it does mandate correct management and timely referral. Therefore, the issue is not whether there is a perinatologist, but whether the process is managed correctly and on time.<\/p>\n<p>It is necessary to speak frankly. If there really was no timely intervention, this is unacceptable. But it is equally clear that it is not right to talk about this situation on social media.<\/p>\n<p>There is a limit in medicine. Patient confidentiality is protected, the colleague is not publicly accused and the process is first evaluated internally. This is not a preference, it is the basis of the profession. Public disclosure of an unproven process poses a privacy risk for the patient, leads to irreversible reputational damage for the colleague and damages trust in the institution. Most importantly, it moves a medical issue from the ground of data to the ground of perception.<\/p>\n<p>However, I cannot ignore this. If a physician feels the need to say this, there is a problem in that system.<\/p>\n<p>The reality on the ground is clear. Many centers in Turkey, especially in the provinces, work with limited staff and sometimes provide services with a single specialist. However, Article 42\/A of the Regulation on the Operation of Inpatient Treatment Institutions defines on-call duty for places with more than one specialist. The 12th Chamber of the Council of State has also clearly stated that an on-call shift cannot be applied in a unit with a single specialist (2020\/5242 E., 2021\/3790 K.). In other words, a single-physician system is already working at the limit. The problem becomes inevitable when a burden is imposed on it contrary to the legislation.<\/p>\n<p>However, this does not justify delay. Difficult circumstances cannot justify delay. This profession does not tolerate \u201cI'll look tomorrow\u201d.<\/p>\n<p>It is therefore necessary to go back to the right questions. What happened to that patient, who was called and when, who intervened and when, and if referral was necessary, was it delayed? Any discussion without clarifying these questions is not healthy.<\/p>\n<p>In conclusion, I make it clear that if there is a delay in the process, this is wrong. It is also wrong to describe this process on social media. The right thing is to examine the process through records, clarify responsibility and correct the system.<\/p>\n<p>My position is clear. Patient safety comes first. But medicine is practiced through records and accountability, not social media. Unless these two principles are protected together, neither the system will be fixed nor trust restored.<\/p>","protected":false},"excerpt":{"rendered":"<p>If there is a delay in the process, this is wrong.
